What is the process for creating a Squeeze Page with an Email and Name field that autoresponds?

How can I set up an autoresponder for a Squeeze Page with an Email and Name field so that when a person completes the form, they receive an email thanking them for doing so? I have attempted to create an autoresponder using PHP, but when I click the “Send” button on the form, it redirects me to the PHP page with all the codes.

How can I create a MailChimp Squeeze Page using HTML?

Does anyone know of a HTML template with integrated MailChimp support that I can use to create a Squeeze Page? I am not looking for something too complex, something like Nature Effect HTML Squeeze Page Template | Webbzo would be great. I need to be able to replace the dummy info with my own and integrate my MailChimp list into the page.
