What is the most effective technique for using video for lead generation purposes? Do you append some strings to the video when you upload it, such as “- namesite.com”? Do you use YouTube annotations?

How can I create a MailChimp Squeeze Page using HTML?

Does anyone know of a HTML template with integrated MailChimp support that I can use to create a Squeeze Page? I am not looking for something too complex, something like Nature Effect HTML Squeeze Page Template | Webbzo would be great.
